Home visits for clients that require medical care/assistance within the household
Family Child Outreach Program:
- Provides support to individuals and families who are affected by FASD
- Offer a mentor to provide support on a one to one basis for up to three years
- Assist in setting goals for client
- Refer clients to necessary services
Health Education Program:
- Supports and educates individuals on healthy living and diabetes
- Dietitian
- Retinal Photographer
- SLICK Clinics (once a month)
Medical Transportation:
Transport clients to and from medical appointments
*require 48 hours advanced notice*

Supplements costs for medical services that are not covered through FNIHB or insurance
