Be part of a team’s who’s primary values include integrity, compassion and innovation. As this is a new program, we are looking for a diplomatic, multi-tasker, who is able to work in a fast-paced environment and work collaboratively with the wrap around team.
The peer support/outreach worker consults with the Social Well-Being Wrap Around Client Liaison (WACL), the clients and families to connect them with specific supports in the community and often off-reserve resources as well. Through their own lived experience(s) with addictions, homelessness and/or mental health illnesses, the Peer Support/Outreach Worker – offers hope to clients by sharing their story, offering mentorship and support; advocating for client needs; and supporting an integrated and harm reduction oriented experience.
These can include, but are not limited to, cultural activities, outside organizational supports, and agencies that assist in navigating the health and/or recovery systems. Using a culturally informed model that is rooted in harm reduction, the Peer Support/Outreach Worker walks alongside clients in their self-identified and self-determined goals for after-care, healing, culture, and community integration; while advocating for access and rights as needed.
DUTIES AND RESPONSIBILITES:
· Attend Wrap Around Client Liaison meetings and assist the outreach team to manage complex client case plans
· May assist clients to appointments, liaising with community and Tsuut’ina programs, supporting the wrap around team, and works side-by-side with the Client Liaison Worker.
· Experience navigating paths to healing and work with clients using culturally informed strategies
· Knowledge of the holistic addictions model including stages of change and harm reduction, cultural pillars of healing and cultural forms of medicine
· As a Peer Support Worker, you will provide interventions and supports that are complementary to the work of a multi-disciplinary team within Addiction & Mental Health.
· The team functions together to provide treatment, rehabilitation and support services for clients with addiction and/or mental illness.
· Ability to effectively and safely resolve crisis situations
· Conduct regular home visits; actively involve the client in planning, goal setting and decision-making
· Ability to work both independently and collaboratively as part of a team
· Ability to establish workload priorities, adjust to new or unexpected events, problem solve, and deal effectively with conflict situations
· Physical ability to perform the duties of the position
· Perform other duties as assigned, including but not limited to reporting
· You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients.
· You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients’ behaviors, participation and ability to function independently to the interdisciplinary team.
· Through your own lived experience(s) with addiction and/or mental illness, and experiences of recovery and resilience, as the Peer Support Worker, you can authentically inspire hope to clients by sharing your recovery story, offering mentorship and recovery support; advocating for client needs; and supporting an integrated and recovery-oriented treatment experience.
· Lived experience with addictions, homelessness and/or mental health illness (must identify in cover letter). Must be willing to share one’s personal experience(s).
· Established ability to maintain healthy boundaries is necessary.
· Knowledge of an Indigenous language & culture.
· Knowledge of systemic issues such as poverty and stigma.
· Knowledge and experience of the principles and practice of harm reduction. Working with a multidisciplinary team.
· Minimum two years’ experience working with diverse and marginalized populations, particularly the homeless (work or volunteer).
· Minimum education and experience includes:
o One year of recent, related work experience with an emphasis on mental health, addictions, homelessness and/or social services
o Or an equivalent combination of education, training and experience
o ASIST Training
o First Aid Mental Health
o Current First Aid and CPR certification; Naloxone Certification
· Works well with community members
· Self-motivated and ability to problem solve with tact, cooperation, and persistence
· Drivers License & Reliable Transportation
· Criminal record check
